      Instructions for Drawings

      The application shall include a plan or drawing. The drawing shall be to scale and must include a title box, scale, name, address/location, north arrow, outlined limits/dimension size shown for all proposed surface openings including the roadway, driveway resurfacing or overlays, etc. The location shall show the location of the work to be done in relation to the outstanding features of the roadway, such as property lines, intersection, pavement lines, sidewalks, trees, drainage structure, utility poles by number, and the character and extent of the work.

        Conditions

        All Trench Permits, Street Opening, Water / Sewer Service Permits shall include the following conditions:

        1. Copies of the Trench Permit, Street Opening/ Access-way Permit and Water  / Sewer Service permits must be onsite at all times work is being done.
        2. Contractor shall contact the DPW at least 24 hours prior to starting work.
        3. Contractor shall contact the DPW for an inspection of the installation / backfill prior to paving.
        4. Contractor shall contact the DPW for final approval after all work is complete.
        5. Contractor shall comply with all State and Federal Safety Requirements
        6. Contractor shall coordinate with Ayer Police Department for traffic management, signage and police details. Failure to provide adequate signage or traffic management will result in revoking of permits.
