The following is the Building Permit Application for Ayer, MA. Listed below are additional forms that may/may not be required in conjunction with the Building Permit. Please fill out all forms that apply and submit them.

After you have completed the form, you will need to:

  • Upload required documents, such as Workman’s Comp and General Liability Insurance
  • Home Improvement Registry License
  • Construction Supervisor License
  • Copy of the signed work agreement
  • Plans

The applicant is responsible for obtaining signatures from the Ayer Water Department and the Tax Collectors Office.


Permits will not be processed unless we have received all necessary documents and payment.  Please submit separate checks for electrical and plumbing permits.  You will be able to upload documents on the next screen.
