The following is the Building Permit Application for Ayer, MA. Listed below are additional forms that may or may not be required in conjunction with the Building Permit.

After you have completed the form, you will need to upload the required documents.
  • Workman’s Comp. and General Liability Insurance
  • Home Improvement Registry License
  • Construction Supervisor License
  • Copy of signed work agreement/contract
  • Plans

The applicant is responsible for obtaining signatures from the Ayer Water Department and the Tax Collectors Office.
Permits will not be processed until we have received all necessary documents and payments. Please submit separate checks for electrical and plumbing permits.
